The 28-year-old, who had one year remaining on his contract, attracted interest from several Championship clubs following a successful loan spell at Derby County last season.
Phillips will now link up with the Baggies' new head coach, Ryan Mason, at The Hawthorns.
The move brings an end to a nine-year stay at Anfield, having joined Liverpool from the Bolton Wanderers academy in 2016.
The centre-back made his debut in 2020 in an FA Cup tie against Everton and went on to make 20 appearances during the following campaign, stepping up during Jurgen Klopp's injury-hit season to help the Reds finish third and achieve Champions League qualification.
His final appearance for Liverpool came against Wolverhampton Wanderers in January 2023.
